			<p style="text-align: justify;">Le batteriocine sono un gruppo di peptidi antimicrobici prodotti dai batteri, in grado di controllare i batteri clinicamente rilevanti e resistenti ai farmaci. Rappresentano una potenziale alternativa farmacologica per sostituire gli attuali antibiotici per il trattamento di malattie causate da batteri resistenti. Come sappiamo, il mondo sta affrontando un aumento significativo delle infezioni causate da agenti infettivi resistenti ai farmaci. Per questo motivo, le batteriocine sono ottime candidate per essere utilizzate come agenti terapeutici.</p>
<p>I prodotti Sterify sfruttano l&#8217;attività antimicrobica della nisina, un peptide antibatterico policiclico prodotto dal batterio Lactococcus lactis che viene utilizzato come conservante alimentare. In questo articolo abbiamo voluto condividere alcuni fatti interessanti su questa nuova classe di agenti antimicrobici, esplorando e sintetizzando una <a href="https://www.frontiersin.org/articles/10.3389/fmicb.2021.630695/full" target="_blank" rel="noopener">panoramica recente pubblicata sulle frontiere</a>.</p>
<ol>
<li>Le batteriocine furono scoperte per la prima volta da André Gratia nel 1925. <li>Come previsto, le batteriocine hanno un record eccezionale per uccidere o ridurre agenti patogeni e patogeni resistenti ai farmaci durante le valutazioni in vitro (Fuchs et al., 2011; Cui et al., 2012; Gabrielsen et al., 2014; Ishibashi et al., 2014 ; Al Atya et al., 2016; Jiang et al., 2017; Aguilar-Pérez et al., 2018; Ansari et al., 2018; Denkovskienė et al., 2019; Peng et al., 2019; Newstead et al. , 2020).</li>
<li>Sebbene sia un dato di fatto che l&#8217;attuale letteratura sulle batteriocine prodotte da batteri Gram-negativi sia dominata dalle batteriocine verso i batteri Gram-positivi (Jamali et al., 2019), esiste una quantità accettabile di batteriocine segnalate per avere una forte attività contro Gram -batteri negativi, inclusi i ceppi patogeni, ad esempio il gruppo piocina di tipo S di P. aeruginosa (Ghequire e De Mot, 2014), o i gruppi microcina e colicina che sono ampiamente riportati per E. coli.</li>
<li>A differenza degli antibiotici, le batteriocine possono essere progettate per attaccarsi in qualsiasi punto della membrana cellulare esterna perché non hanno un recettore specifico (Bonhi e Imran, 2019) e possono essere prodotte in situ dai probiotici (Dobson et al., 2012; O&#8217; Shea et al., 2012).</li>
<li>Le batteriocine sono classificate in diversi modi, inclusa la produzione di ceppi, i comuni meccanismi di resistenza e il meccanismo di uccisione. I metodi di uccisione includono: formazione di pori, attività nucleasica, inibizione della produzione di peptidoglicani. Il sistema di denominazione è problematico per diversi motivi. Inoltre, nelle piante sono state prodotte batteriocine attive contro E. coli, Salmonella e Pseudomonas aeruginosa con l&#8217;obiettivo di utilizzarle come additivi alimentari. Inoltre, è stato recentemente dimostrato che le batteriocine attive contro i batteri patogeni delle piante possono essere espresse nelle piante per fornire una robusta resistenza alle malattie delle piante.</li>
<li>Secondo l&#8217;Antibacterial Agent in Preclinical Development Book del 2019 dell&#8217;OMS, 27 dei 252 agenti antimicrobici in stato di revisione preclinica sono considerati peptidi antimicrobici. In uno studio indipendente, Theuretzbacher et al. (2020) hanno identificato l&#8217;attuale pipeline antibatterica globale e hanno scoperto che 135 dei 407 progetti preclinici di 314 istituzioni private e pubbliche erano correlati alla produzione di peptidi antimicrobici sintetici e naturali, prodotti naturali e inibitori dell&#8217;LpxC e la maggior parte di queste molecole mirano ai Gram-negativi batteri.</li>

			<p>Bacteriocins are a group of antimicrobial peptides produced by bacteria, capable of controlling clinically relevant and drug-resistant bacteria. They represent a potential drug alternative for replacing current antibiotics to treat diseases caused by resistant bacteria. As we know, the world is facing a significant increase in infections caused by drug-resistant infectious agents. For this reason, bacteriocins are strong candidates to be used as therapeutic agents.</p>
<p>Sterify products exploit the antimicrobial activity of nisin, a polycyclic antibacterial peptide produced by the bacterium Lactococcus lactis that is used as a food preservative. In this article we wanted to share some interesting facts about this new class of antimicrobial agents, exploring and summarizing a recent <a href="https://www.frontiersin.org/articles/10.3389/fmicb.2021.630695/full" target="_blank" rel="noopener">overview published on frontiers</a>.</p>
<ol>
<li>Bacteriocins were first discovered by André Gratia in 1925. He was involved in the process of searching for ways to kill bacteria, which also resulted in the development of antibiotics and the discovery of bacteriophage, all within a span of a few years. He called his first discovery “colicine” because it killed E. coli.</li>
<li>Bacteriocins display a non-toxic behavior at in vitro assays (Cebrián et al., 2019). This is also confirmed in vivo and clinically. Numerous researches have examined the biosafety and toxicity of bacteriocins utilizing a variety of administration methods, including oral, intraperitoneal, nasal, and topical. In particular, topical application of bacteriocins has been reported to be successfully tested for skin infection with no toxicity effects.</li>
<li>Bacteriocins can be naturally synthetized by native producers. This mechanism exploits the bacteriocins excretion by dedicated membrane-associated ATP-binding cassette (ABC) transporters or by the general secretion pathway of the cell (Munoz et al., 2011). Bacteriocins can also be naturally synthetized by heterologous producers to increase the bacteriocin production yield from native producers by facilitating the control of gene expression or increasing the production levels. Bacteriocins can be also chemically synthetized.</li>
<li>As expected, bacteriocins have an outstanding record to kill or reduce pathogens and drug-resistant pathogens during in vitro assessments (Fuchs et al., 2011; Cui et al., 2012; Gabrielsen et al., 2014; Ishibashi et al., 2014; Al Atya et al., 2016; Jiang et al., 2017; Aguilar-Pérez et al., 2018; Ansari et al., 2018; Denkovskienė et al., 2019; Peng et al., 2019; Newstead et al., 2020).</li>
<li>Although it is a fact that the current literature for bacteriocins produced from Gram-negative bacteria is dominated by bacteriocins toward Gram-positive bacteria (Jamali et al., 2019), there is an acceptable amount of bacteriocins reported to have a strong activity against Gram-negative bacteria, including the pathogenic strains, e.g., the S-type pyocin group from the P. aeruginosa (Ghequire and De Mot, 2014), or the microcin and colicin groups that are vastly reported for E. coli.</li>
<li>Unlikely to antibiotics, bacteriocins can be engineered to attach anywhere on the cellular outer membrane because they do not have a specific receptor (Bonhi and Imran, 2019) and they can be produced in situ by probiotics (Dobson et al., 2012; O’Shea et al., 2012).</li>
<li>Bacteriocins are categorized in several ways, including producing strain, common resistance mechanisms, and mechanism of killing. Methods of killing include: pore-forming, nuclease activity, peptidoglycan production inhibition. The naming system is problematic for several reasons. First, it would be more accurate to name bacteriocins according to what they kill if their killing spectrum coincided with genus or species names. However, the bacteriocins frequently possess spectra that exceed the bounds of their named taxa and almost never kill the majority of the taxa for which they are named. Additionally, the bacteriocin&#8217;s initial name usually comes from the organism that produces it rather than the specific strain that it kills. Because of this, using this name system as a theoretical foundation is challenging.</li>
<li>As of 2016, nisin was the only bacteriocin generally recognized as safe by the FDA and was used as a food preservative in several countries. Furthermore, bacteriocins active against E. coli, Salmonella and Pseudomonas aeruginosa have been produced in plants with the aim for them to be used as food additives. Moreover, it has been recently demonstrated that bacteriocins active against plant pathogenic bacteria can be expressed in plants to provide robust resistance against plant disease.</li>
<li>According to 2019 WHO’s Antibacterial Agent in Preclinical Development Book, 27 of 252 antimicrobial agents in preclinical revision status are considered as antimicrobial peptides. In an independent study, Theuretzbacher et al. (2020) identified the current global antibacterial pipeline and found that 135 of 407 preclinical projects from 314 private and public institutions were related to producing synthetic and natural antimicrobial peptides, natural products, and LpxC inhibitors, and most of these molecules are targeting Gram-negative bacteria.</li>
<li>The future of this new class of therapeutic agents is intriguing and may lead to novel discoveries. Fields et al. (2020) were the first to design the very first fully de novo bacteriocin by using a machine-learning approach. Acuña et al. (2012) were able to design chimeric bacteriocins that retained the properties to kill both Gram-positive and Gram-negative bacteria. Moreover, other authors have preferred to repurpose the bacteriocins by exploiting their capability against tumor cells (Varas et al., 2020).</li>

			<p style="text-align: justify;"><strong>Malattia parodontale</strong>, conosciuta anche come <em>gum disease</em>, è un&#8217;infezione <em>grave</em> che colpisce i tessuti molli dell&#8217;apparato di supporto del dente. La maggior parte della popolazione adulta può soffrire di parodontite, che può distruggere la struttura ossea che supporta i denti se non trattata, portando così ad allentamento o addirittura perdita di denti.</p>
<p>Questa è una condizione comune, ma prevenibile. È il risultato di una scarsa igiene orale che porta alla formazione di un complesso biofilm batterico che colonizza le superfici dentali che inizia come infiammazione gengivale e poi si estende al legamento parodontale e all&#8217;osso alveolare. I sintomi includono gonfiore, arrossato, gengive soft-touch che sanguinano facilmente, alito cattivo, e masticazione dolorosa. Quando si verificano questi segni, è consigliabile visitare il dentista il più presto possibile. Clinicamente, la lesione può essere vista come un &#8220;<strong>tasca parodontale</strong>&#8220;, che per semplicità può essere descritto come un distacco del tessuto gengivale dal dente, il che significa che la gomma è appoggiata sul dente e non più attaccata ad esso. Il microambiente della tasca è ideale per la proliferazione di batteri aggressivi in grado di perpetuare i danni nel tempo.</p>
<p>La misurazione della tasca parodontale attraverso una sonda, il rilevamento dell&#8217;indice di sanguinamento e, in alcuni casi, la tipizzazione batterica con sonde DNA sono i principali indicatori diagnostici per la gravità della patologia. In un parodonto sano, non c&#8217;è distacco dell&#8217;epitelio gengivale dal dente e lo spazio tra i due non è inferiore a 2 mm di profondità e l&#8217;indice di sanguinamento è quasi zero.</p>
<p>I trattamenti disponibili includono terapia non chirurgica immediata, ad esempio, sessioni di igiene orale con ultrasuoni e flusso d&#8217;aria con glicina o eritritolo per abbattere il biofilm batterico sopraagingivo e decontaminazione delle tasche parodontali attraverso debridement superficiale della radice (RSD) e la scala e la piallatura delle radici (SRP) per rimuovere la placca subgengivale e tartaro. L&#8217;obiettivo è quello di rimuovere meccanicamente il fattore causale della parodontite e quindi promuovere la guarigione naturale della gengiva e la riduzione delle tasche parodontali. Tuttavia, tali manovre sono a volte limitate dalla profondità e dalla complessità anatomica della tasca e la nuova colonizzazione batterica può infiltrarsi e re-infettare le tasche dopo RSD e SRP, inibendo il processo di guarigione.</p>
<p>I trattamenti combinati con RSD e SRP sono stati sviluppati per migliorare la guarigione gengivale e prevenire le infezioni ricorrenti e l&#8217;infiammazione delle tasche parodontali. Alcuni di questi metodi includono la terapia laser antimicrobica attraverso l&#8217;attivazione di una molecola fotosensibile (<em>terapia fotodinamica</em>), o l&#8217;uso di antibiotici o molecole antimicrobiche consegnati direttamente in tasche parodontali per ridurre il carico batterico e prevenire il ri-infezione. La consegna del farmaco viene effettuata attraverso l&#8217;uso di collutori in pasta o ponteggi di rilascio, che possono essere posizionati nelle tasche con la permanenza di giorni o settimane. Gli antibiotici, oltre ad essere usati localmente, possono anche essere amministrati sistematicamente, tuttavia, c&#8217;è il rischio di causare resistenza agli antibiotici, che, oltre ad essere uno dei maggiori rischi per la salute per la popolazione mondiale, può peggiorare il quadro clinico del paziente. <p><strong>La nostra strategia per evitare l&#8217;uso di antibiotici migliorando il trattamento SRP gold standard di parodontite è Sterify gel</strong>. È un idrogel polimerico mucoadesivo in una siringa pronta all&#8217;uso preriempita. Può essere applicato all&#8217;interno della tasca parodontale o periimplantare dopo la procedura di igiene SRP. Nella tasca parodontale, Sterify Gel agisce principalmente come riempitivo per ripristinare il volume impedendo fisicamente ai batteri di entrare e reinfezionare la tasca stessa. 			<p style="text-align: justify;"><strong>Periodontal disease</strong>, also known as <em>gum disease</em>, is a <em>serious infection</em> that affects the soft tissues of the supporting apparatus of the tooth. Most of the adult population can suffer from periodontitis, which can destroy the bone structure that supports the teeth if untreated, thus leading to loosening or even loss of teeth.</p>
<p>This is a common, yet preventable condition. It is the result of poor oral hygiene that leads to the formation of a complex bacterial biofilm that colonizes tooth surfaces that starts as gum inflammation and then extends to the periodontal ligament and alveolar bone. Symptoms include swollen, reddened, soft-touch gums that bleed easily, bad breath, and painful chewing. When these signs occur, it is advisable to visit the dentist as soon as possible. Clinically, the lesion can be seen as a &#8220;<strong>periodontal pocket</strong>&#8220;, which for simplicity can be described as a detachment of the gum tissue from the tooth meaning that the gum is resting on the tooth and no longer attached to it. The microenvironment of the pocket is ideal for the proliferation of aggressive bacteria capable of perpetuating the damage over time.</p>
<p>The measurement of the periodontal pocket through a probe, the detection of the bleeding index and, in some cases, the bacterial typing with DNA probes are the main diagnostic indicators for the severity of the pathology. In a healthy periodontium, there is no detachment of the gingival epithelium from the tooth and the space between the two is not less than 2 mm deep and the bleeding index is almost zero.</p>
<p>Available treatments include immediate non-surgical therapy, e.g., oral hygiene sessions with ultrasound and air-flow with glycine or erythritol to break down the supragingival bacterial biofilm and decontamination of periodontal pockets through root surface debridement (RSD) and scaling and root planing (SRP) to remove subgingival plaque and tartar. The goal is to mechanically remove the causative factor of periodontitis and thus promote natural healing of the gingiva and reduction of periodontal pockets. However, such maneuvers are sometimes limited by both the depth and anatomical complexity of the pocket and new bacterial colonization can infiltrate and re-infect the pockets after RSD and SRP, inhibiting the healing process.</p>
<p><strong>Combination treatments with RSD and SRP have been developed to improve gingival healing and prevent recurrent infection and inflammation of periodontal pockets</strong>. Some of these methods include antimicrobial laser therapy through activation of a photosensitive molecule (<em>photodynamic therapy</em>), or the use of antibiotics or antimicrobial molecules delivered directly into periodontal pockets to reduce the bacterial load and prevent re-infection. Drug delivery is done through the use of paste mouthwashes or release scaffolds, which can be placed in the pockets with the permanence of days or weeks. Antibiotics, in addition to being used locally, can also be administered systemically, however, there is a risk of causing antibiotic resistance, which, in addition to being one of the major health risks for the world&#8217;s population, can worsen the patient&#8217;s clinical picture. We have explored in our previous articles how antimicrobial resistance is an emergent healthcare threat that may doom our lives if not properly tackled. Check them out (link).</p>
<p><strong>Our strategy to avoid the use of antibiotics while improving the SRP gold standard treatment of periodontitis is Sterify gel</strong>. It is a mucoadhesive polymeric hydrogel in a pre-filled ready-to-use syringe. It can be applied inside the periodontal or peri-implant pocket after the SRP hygiene procedure. In the periodontal pocket, Sterify Gel acts primarily as a filler to restore volume by physically preventing bacteria from entering and re-infecting the pocket itself. The medical device can thus mechanically protect the treated pockets and bone defects, promoting tissue healing.  The occluding action at the level of periodontal and peri-implant gingival pockets, in synergy with the accessory action of hydroxytyrosol, nisin, and magnesium ascorbyl phosphate, creates an unsuitable environment for bacterial growth and promotes subsequent tissue regeneration. We have successfully ended the pre-clinical assessment, showing Sterify Gel biocompatibility, and started a clinical investigation according to <strong>Medical Device Regulation (EU) 2017/745</strong>.</p>

<p><strong>1. PIÙ VECCHIO DEI DINOSAURI</strong><br />
I batteri esistono da più di tre miliardi di anni: sono i più antichi Lifeform del pianeta. Per capire la portata di questo fatto, sappiate che la specie umana ha più di 12.000 anni (dalla rivoluzione neolitica), l&#8217;estinzione dei dinosauri è avvenuta 65,95 milioni di anni fa, e l&#8217;età della Terra è di circa 4,5 miliardi di anni.</p>
<p><strong>2. VELOCE COME UN FULMINE</strong><br />
Un batterio può solitamente muoversi cento volte la dimensione del suo corpo in un secondo. Per vederlo in prospettiva: un pesce si muove dieci volte la lunghezza del suo corpo nello stesso tempo!</p>
<p><strong>3. I BATTERI SONO BUONI DA MANGIARE</strong><br />
Escludendo la contaminazione indesiderabile, i batteri sono trovati in numerosi alimenti poiché sono utilizzati nella preparazione di molti alimenti. Esempi sono la lievitazione del pane e dei derivati, la fermentazione alcolica, il formaggio e molti altri prodotti che aiutano la flora batterica intestinale come lo yogurt.</p>
<p><strong>4. DIMENSIONI GALATTICHE</strong><br />
Ci sono 5000 miliardi di miliardi di batteri sulla Terra, una quantità molto più grande del numero di stelle nell&#8217;Universo. Se li allineiamo uno per uno (ognuno è grande un micron&#8230;), otterremmo una distanza di 530 milioni di anni luce.</p>
<p><strong>5. SONO QUASI COMPLETAMENTE INNOCUI</strong><br />
Ci sono più batteri nel nostro corpo che cellule umane. Ci aiutano con la digestione e per difenderci dai batteri più cattivi. Considerando tutti i batteri conosciuti, meno dell&#8217;1% può essere dannoso e farci ammalare.</p>
<p><strong>6. SONO STATI SCOPERTI NEL 1674</strong><br />
Antonie van Leeuwenhoek &#8211; uno scienziato olandese &#8211; ha osservato per la prima volta i batteri in un campione ottenuto raschiando la bocca umana con un microscopio di nuova invenzione. Li chiamava animalcula, che significa &#8220;piccoli animali&#8221;. Il nome bacterium deriva dalla parola greca βακτήριου e significa &#8220;bastone&#8221;; fu coniato nel 1839 dal naturalista tedesco Gottfried Ehrenberg.</p>
<p><strong>7. HANNO SOLTANTO UNA CELLA, TUTTAVIA&#8230;. </strong><br />
I batteri sono organismi unicellulari, cioè hanno una sola cellula. Tuttavia, quando si riproducono possono formare aggregati complessi (ad es. producendo biofilm), esibendo un comportamento coordinato e ottenendo vantaggi biologici rispetto a una singola unità.</p>

<p><strong>10. SI ADATTANO RAPIDAMENTE</strong><br />
I batteri si adattano rapidamente, rendendo difficile trovare un antibiotico che funziona in modo coerente. L&#8217;uso di antibiotici spinge i batteri a diventare resistenti, rendendo inefficaci le terapie consolidate e aumentando il rischio di morte per infezione.</p>

			<p>Although they are invisible to our eyes, bacteria are everywhere: inside our bodies and outside in the environment. In this article, we are going to tell some interesting facts:</p>
<p><strong>1. OLDER THAN DINOSAURS</strong><br />
Bacteria have existed for more than three billion years: they are the oldest lifeform on the planet. To understand the scale of this fact, know that the human species is more than 12,000 years old (since the Neolithic revolution), dinosaurs’ extinction happened 65.95 million years ago, and Earth’s age is about 4.5 billion years.</p>
<p><strong>2. FAST AS LIGHTNING</strong><br />
A bacterium can usually move a hundred times the size of its body in one second. To see it in perspective: a fish moves ten times the length of its body in the same time!</p>
<p><strong>3. BACTERIA ARE GOOD TO EAT</strong><br />
Excluding undesirable contamination, bacteria are found in numerous foods since they are used in the preparation of many foods. Examples are the leavening of bread and derivatives, alcoholic fermentation, cheese, and many other products that help the intestinal bacterial flora such as yoghurt.</p>
<p><strong>4. GALACTIC SIZES</strong><br />
There are 5000 billion billion billion bacteria on Earth, a quantity much larger than the number of stars in the Universe. If we align them in a row, one by one (each one is one micron big&#8230;), we would obtain a distance of 530 million light-years.</p>
<p><strong>5. THEY ARE ALMOST COMPLETELY HARMLESS</strong><br />
There are more bacteria in our bodies than human cells. They help us with digestion and to defend us from the nastier bacteria. Considering all known bacteria, less than 1% can be harmful and make us sick.</p>
<p><strong>6. THEY WERE DISCOVERED IN 1674</strong><br />
Antonie van Leeuwenhoek &#8211; a Dutch scientist &#8211; observed bacteria for the first time in a sample obtained by scraping the human mouth with a newly invented microscope. He called them animalcula, which means &#8220;small animals&#8221;. The name bacterium comes from the Greek word βακτήριου and means &#8220;stick&#8221;; it was coined in 1839 by the German naturalist Christian Gottfried Ehrenberg.</p>
<p><strong>7. THEY ONLY HAVE ONE CELL, HOWEVER&#8230;.</strong><br />
Bacteria are unicellular organisms, i.e., they only have one cell. However, when they reproduce they can form complex aggregates (e.g. by producing biofilm), exhibiting coordinated behavior and gaining biological advantages over a single unit.</p>
<p><strong>8. THEY HAVE A UNIQUE SHAPE</strong><br />
Bacteria are often shaped like a sphere or a stick. However, they also have other bizarre shapes, such as a comma- or corkscrew-shape.</p>
<p><strong>9. THEY ARE TOUGH</strong><br />
Bacteria can survive in the most hostile environments, such as glaciers or the depths of the sea. Through the use of spores, they can survive in extreme conditions, e.g., 122°C in hydrothermal springs.</p>
<p><strong>10. THEY ADAPT QUICKLY</strong><br />

			<p>L&#8217;allarme dei batteri resistenti ai farmaci minaccia la nostra società (ed è anche una ragione per cui abbiamo fondato questa società). Per capire una certa misura di questa emergenza, ecco una lista di fatti interessanti sugli antibiotici.</p>
<p><strong>1. GLI ANTIBIOTICI NON SONO SEMPRE UTILI</strong><br />
Gli antibiotici sono molto potenti, possono uccidere i batteri che causano infezioni e malattie, ma non funzionano per tutto. Nella maggior parte dei casi, non sono utili per il raffreddore, l&#8217;influenza, o qualsiasi altra malattia causata da un virus. Inoltre, non esiste una taglia unica che si adatti a tutti: ci sono centinaia di antibiotici con diversi meccanismi di azione. Prendendo l&#8217;antibiotico sbagliato non ha efficacia e potrebbe causare effetti collaterali dannosi. L&#8217;abuso di antibiotici guida anche la resistenza agli antibiotici, che indebolisce anche gli antibiotici efficaci. Questi farmaci sono davvero mutageni: quando non uccidono ogni batterio, scatenano altre cellule a lottare per la sopravvivenza. <p><strong>2. EFFETTI COLLATERALI ANTIBIOTICI: SISTEMA IMMUNITARIO PIÙ DEBOLE E AUMENTO DEL RISCHIO DI OBESITÀ</strong><br />
Gli antibiotici, anche se efficaci, hanno diversi effetti collaterali che includono: diarrea, reazioni allergiche come eruzioni cutanee, vesciche cutanee, gonfiore del collo e della gola, difficoltà di respirazione, e &#8211; ultimo ma non meno importante -resistenza agli antibiotici. <p><strong>3. SEMPRE PIÙ PERSONE MUOIONO DI RESISTENZA AGLI ANTIBIOTICI</strong><br />
La resistenza agli antibiotici è un fenomeno naturale; tuttavia, il suo uso eccessivo rende il problema molto peggiore. Poiché i batteri sono inclini a mutare e a sviluppare resistenza, è importante usarli con parsimonia e solo quando necessario per mantenere la loro efficacia. Secondo il rapporto dell&#8217;Organizzazione Mondiale della Sanità 2019, ogni anno muoiono quasi 700.000 persone a causa della malattia resistente ai farmaci. Se questo problema non sarà affrontato correttamente, il numero potrebbe crescere a 10 milioni all&#8217;anno entro il 2050. All&#8217;interno di questa proiezione, la resistenza agli antibiotici risulterebbe più letale del cancro. Molti importanti progressi medici saranno spazzati via e le persone rischieranno la morte per semplici infezioni.</p>
<p><strong>4. USO DI ANTIBIOTICI NEL BESTIAME ANCHE GUIDARE RESISTENZA</strong><br />

L&#8217;aumento del benessere degli animali e il divieto di sistemi intensivi possono fornire una soluzione sostenibile.</p>
<p><strong>5. LA SCOPERTA DI NUOVI ANTIBIOTICI HA RALLENTATO</strong><br />
Con l&#8217;aumento della resistenza agli antibiotici in tutto il mondo, trovare nuovi ed efficaci antibiotici sta diventando ogni giorno più difficile. Il processo di ricerca di nuovi farmaci è lento e costoso: dieci anni fa, ogni dollaro investito in R&amp;D avrebbe portato a dieci centesimi; oggi, ogni dollaro messo in R&amp;D genera due centesimi di ritorno dell&#8217;investimento. Inoltre, la resistenza agli antibiotici sta accelerando. La vancomicina è stata prescritta per la prima volta nel 1972, e poi i batteri resistenti alla vancomicina sono emersi nel 1988. La daptomicina è uscita nel 2003, con una resistenza notevole nel 2004. Con i sistemi basati sull&#8217;intelligenza artificiale, la ricerca di nuovi farmaci diventerà più efficace, più veloce e più economica, ma la lotta contro la resistenza agli antibiotici richiederà non solo strategie multidisciplinari, ma anche un cambiamento di abitudini e nuove normative.</p>

<p><strong>1. Antibiotics are not always useful</strong><br />
Antibiotics are very powerful, they can kill bacteria that cause infections and diseases, but they do not work for everything. In most cases, they are not useful for colds, flu, or any other disease caused by a virus. In addition, there is no one size fits all: there are hundreds of antibiotics with different mechanisms of action. Taking the wrong antibiotic has no effectiveness and could cause harmful side effects. Antibiotic abuse also drives antibiotic resistance, which also weakens effective antibiotics. These drugs are really mutagenic: when they don’t kill every bacterium, they trigger other cells to fight for survival. Resistant bacteria can pass their code to their offspring, increasing the number of antibiotic-resistant cells. In addition, resistances can accumulate resulting in &#8220;superbugs&#8221; being immune to multiple antibiotics.</p>
<p><strong>2. ANTIBIOTIC SIDE EFFECTS: WEAKER IMMUNE SYSTEM AND INCREASED RISK OF OBESITY</strong><br />
Antibiotics, although effective, have several side effects that include: diarrhea, allergic reactions such as rashes, skin blisters, swelling of the neck and throat, difficulty breathing, and &#8211; last but not least -antibiotic resistance. Antibiotics fight all kinds of bacteria without distinguishing the virtuous bacteria in your body. Most of them are harmless and good for you, even helping your immune system stay healthy and strong. Overuse and overuse of antibiotics weaken immune cells making them more susceptible to reinfection. In addition, antibiotics also stop bacteria in the gastrointestinal tract. This can permanently alter the bacterial balance and absorption capacity of the intestine, increasing the risk of obesity.</p>
<p><strong>3. MORE PEOPLE DIE OF ANTIBIOTIC RESISTANCE</strong><br />
Antibiotic resistance is a natural phenomenon; however, its overuse makes the problem much worse. Since bacteria are prone to mutate and develop resistance, it is important to use them sparingly and only when necessary to maintain their effectiveness. According to the 2019 World Health Organization report, nearly 700,000 people die each year from drug-resistant disease. If this problem is not addressed properly, the number could grow to 10 million per year by 2050. Within this projection, antibiotic resistance would be more lethal than cancer. Many important medical advances will be wiped out and people will risk death from simple infections.</p>
<p><strong>4. USE OF ANTIBIOTICS IN LIVESTOCK ALSO DRIVE RESISTANCE</strong><br />
There is abuse of antibiotics and abuse even in farms to improve the growth of livestock and to combat their infections. Almost 66% of antibiotics are used for animals, not for people (world data). With the increase in antibiotic resistance worldwide, finding new and effective antibiotics is becoming increasingly difficult. The process of finding new drugs is slow and expensive: ten years ago, every dollar invested in R&amp;D would have led to ten cents; today, every dollar put in R&amp;D generates two cents of return on investment. In addition, antibiotic resistance is accelerating. Vancomycin was first prescribed in 1972, and then vancomycin-resistant bacteria emerged in 1988. Daptomycin came out in 2003, with considerable resistance in 2004. With systems based on artificial intelligence, the search for new drugs will become more effective, faster and cheaper, but the fight against antibiotic resistance will require not only multidisciplinary strategies, but also a change of habits and new regulations.</p>

<p>Numero di decessi e costi sanitari correlati alla resistenza antimicrobica ora e nel 2050 se non viene intrapresa alcuna azione:</p>
<p>700.000 persone in tutto il mondo muoiono ogni anno a causa di infezioni resistenti ai farmaci.</p>
<p>Se non viene intrapresa alcuna azione, le malattie resistenti ai farmaci potrebbero causare 10 milioni di morti ogni anno entro il 2050.</p>

<p>2/3 di questo costo sanitario supplementare sarà a carico dei paesi a basso e medio reddito attraverso la tubercolosi multi-resistente ai farmaci, per esempio, o in mancanza di trattamenti per la polmonite.</p>
<p>Numeri relativi al drammatico fallimento dell&#8217;innovazione nel campo degli antibiotici:</p>
<p>L&#8217;approvazione <strong>degli antibiotici</strong> è caduta a un misero <strong>un ottavo</strong> di quello che era 30 anni fa.</p>
<p>L&#8217;ultima grande nuova classe di antimicrobici è stata scoperta tre decenni fa .</p>
<p>Il numero di brevetti relativi agli antibiotici depositati a livello globale è diminuito da 8600 negli anni 2004 &#8211; 2005 a circa 5000 ora.</p>
<p>Il numero di grandi aziende farmaceutiche attive in questo settore è diminuito da 18 nel 1990 a 6 nel 2016.</p>
<p>È interessante notare che la resistenza antimicrobica non è guidata solo dall&#8217;abuso e dall&#8217;abuso di antibiotici nell&#8217;uomo, ma anche dal suo uso nel bestiame e dal suo inquinamento ambientale.</p>
